Authorization for Release of Photographs for Educational and Publicity Purposes

I/we, the undersigned, hereby authorize North Carolina State University and the North Carolina Cooperative Extension Service to use photographs, which I/Ie have voluntarily allowed to be taken by University representatives. I/we understand that such use may include, but shall not be limited to, publications, slide shows or displays.

I/we hereby waive any right to which I/we or my/our heirs may otherwise be entitled by law to assert against North Carolina State University or North carolina Cooperative Extension Service on account of injury sustained to my/our reputation arising from causes of action including, but not limited to, libel, slander, defamation of character and invasion of privacy as a result of such publication and hereby release North Carolina State University or North Carolina Cooperative Extension Service from any liability on account of such injury.

I/we fully understand the comprehensive nature of this release and voluntarily consent to sign it.
